About Your Visit

At Urology Associates of North Texas, patient care is our number one priority. We'll work with you to schedule an appointment with the physician of your choice. If that physician isn't available, we'll suggest another qualified member of the group for you to consider.

When you call to schedule, please feel free to confidentially disclose the nature of your medical problem, so we can ensure enough time for your visit. To schedule an appointment online, please fill out one of the forms below.

When you come to the office, please bring the following information:

• A list of medications you are currently taking
  
• Any records or x-rays related to your problem from your primary care or referring physician
  
• Any forms that were mailed to you by our office

The receptionist will assist you in obtaining any x-rays or reports that might be also required.

Your appointment will usually be confirmed one or two days prior to your visit to our office.

We strive to see patients punctually. Please help us expedite the process by completing the patient information form online 24 hours in advance of your appointment or print and complete to bring to your appointment.

New Patients

Appointments can be made via telephone, on the internet or in person. Please have your insurance information available to give to the receptionist if you call. If you are currently covered under a managed care plan, we can schedule your appointment more efficiently if you provide us a physician referral number. (You will need to contact your primary care physician and ask for a referral in advance.)

If for some reason the Urology Associates of North Texas physician that you requested to see is not available, we will try to accommodate your request at the earliest time available or schedule you with another physician.
